What is Medicaid?

In order to facilitate the application process, it's essential to gather comprehensive documentation. This includes providing copies of canceled checks and obtaining full bank statements for all accounts, even if the applicant is listed as an authorized user. Detailed financial records spanning 60 months for each bank account are necessary.

Time sensitivity is paramount in completing these applications. Any requests for additional information must be promptly addressed within the specified timeframe. It's imperative to document all liquidated assets from the past 60 months. Additionally, applicants should be prepared to provide:

  • Written documentation of all cash values associated with insurance policies.

  • Written confirmation of no cash value for insurance policies, including accidental death coverage.

  • Social Security numbers, dates of birth, marriage, and, if applicable, dates of death for both the applicant and spouse (or former spouse).
